Asante Home Care, LLC. is an intermediary home care agency accredited by the Connecticut Department of Social Services. The agency was formed with a vision and a mission of “improving care, one patient at a time.” Through the following programs we will help the most vulnerable remain in private homes and maintain their independence as long as possible.

Adult Family Living/Foster Care Program (AFL):
Adult Family Living (AFL) is a program the helps frail elderly individuals to remain living in private homes instead of moving into assisted living communities or nursing homes. The program works by providing financial assistance and support for family members or friends that act as both primary in-home caregivers and hosts of an elderly individual.

Under AFL, an elderly individual moves into the home of a family member or friend, or the family member or friend moves into the home of the elderly individual. Asante Home Care, LLC provides training, quality oversight, and support for the family member or friend working as the caregiver.
The caregiver receives compensation (a monthly tax-free stipend) for providing the services provided to the elderly. The monthly stipend allocation is based on the Activities of Daily Living, cognitive or behavioral needs and is determined by the CHCPE or PCA assessment completed by the care manager.

Personal Care Attendant (PCA):
This is a Medicaid program which pays for the cost of a personal care attendant to assist the participant with their activities of daily living and instrumental activities of daily living.

We empower our team by mentoring them and providing resources for both professional and personal growth.

Cost of Service:
There is no cost for individuals who meet the Connecticut Home Care Program for Elders (CHCPE) or Personal Care Assistance (PCA) eligibility requirements. However, a list of private fees is available upon requests for individuals who are interested in services but do not meet CHCPE eligibility requirements.
